This Consumer Health Data Privacy Policy outlines how Anytime Dental Implant Centers LLC, along with its subsidiaries and affiliates (collectively referred to as "Company," "we," "us," or "our"), manages the collection, usage, and disclosure of personal information categorized as "consumer health data" under the relevant state laws. This is applicable to our Company-owned and operated websites, digital platforms linked to this policy, and associated communication and marketing channels (collectively referred to as the "Site"). It does not include protected health information provided to the Company in connection with dental or healthcare services. In cases of inconsistency between such notices and this Consumer Health Data Privacy Policy, the Notice of Privacy Practices will take precedence.

  1. COLLECTION OF CONSUMER HEALTH DATA

We gather the following categories of consumer health data directly from consumers via our Site (including our website and call center) when scheduling a consultation appointment:

  • Information about individual health conditions or status, such as dental conditions (e.g., missing teeth, periodontal disease).
  • Details of medical interventions or health-related procedures, such as dental history or treatments (e.g., previous implants, dentures).
  • Data identifying consumers seeking healthcare services, including responses regarding their expectations from dental implants (e.g., to boost confidence or repair an accident/injury), alongside the mentioned data.

We utilize the consumer health data mentioned above to process your consultation appointment requests effectively, to engage with you regarding your dental history and potential interest in dental implants, and to tailor content that might be relevant to you. Additionally, we provide you with information about our products and services. When you use our call center to schedule a consultation, we, along with our service provider, may record or monitor the calls for purposes of quality assurance and training. Furthermore, we may use the collected consumer health data for internal reporting and analytical purposes.

We may share consumer health data with the following entities or individuals to facilitate the requested scheduling services:

  • Related Companies:We might share consumer health data with affiliated companies—those that control, are controlled by, or are under common control with us, and are co-branded. This includes MULTI-SPECIALTY HOLDCO, LLC (MSH), TD Dental LLC, TD Dental Management LLC, TD Dental Holdings LLC mainly for internal reporting and analytical purposes.
  • Service Providers:We may share consumer health data with third-party service providers who perform services for us or on our behalf, such as hosting our website or managing our database, to deliver our consultation scheduling services.
  • Affiliated Dental and Healthcare Practices:We share your contact details and the date and time of your consultation appointment with the Anytime Dental Implant Centers you choose.
  • Business Transaction Participants:In the event of a sale or transfer of our business or assets, or if we are involved in a corporate restructuring, merger, acquisition, dissolution, or similar proceedings, we may transfer your personal information, including consumer health data, to third parties involved in such transactions, including during the negotiation stage.

We do not allow third parties to collect your consumer health data over time across different internet websites or online services when you schedule a consultation through our web form.
